BASIC LAW ENFORCEMENT TRAINING ENROLLMENT FORMS All of the forms listed below are necessary and must be completed before you are officially enrolled in the class. DO NOT PROCRASTINATE ON GETTING YOUR PAPERWORK TO ME. THERE IS A CAP ON STUDENTS FOR THIS CLASS. ENSURE YOUR SEAT BY GETTING YOUR PAPERWORK DONE EARLY. I highly suggest that you buy a 3 ring binder style notebook and place all of the paperwork in that notebook in the order below Beginning in January I will conduct a BLET information briefing each Wednesday night at 7:30 pm. Report to the reception desk at Building 1 for the location of the meeting. This is needed for all students enrolled at Martin Community College. Fill in the information, save, and print. You must complete a Personal History Statement. All questions must be answered truthfully and honestly. This is a fillable form. Fill it out completely, save it, and print a copy. This form will follow you for the rest of your law enforcement career so make sure you keep your copy on the computer. Make sure you complete all questions truthfully. Any falsification or omission will lead to rejection or dismissal. If any information is found to be incorrect, a letter will be sent to the North Carolina Criminal Justice Training and Standards Division stating that you falsified official documents. All persons taking BLET classes must take a reading test called the Comprehensive Adult Skills Assessment Survey (CASAS). This is required by North Carolina Criminal Justice Training and Standards Commission. The test is administered on Mondays at 8 am, 1 pm, and 6 pm. Call before coming to ensure the MCC Learning Lab will be open. Fill out the CASAS Testing Referral Form and report to the MCC Learning Lab (Building 2, Room 21) at least 15 minutes prior to the test. (If you take the test at 6pm, be patient, somebody will show up). There will be a lot of reading required in this course and we need to make sure that you can comprehend the material. You will be required to score at least an 11th grade (246 points or higher) reading level. In the event you score less than 11th grade reading level, don't give up, call the school director. PHYSICAL Fill out the Medical History Statement (Form F-1 Fillable) before seeing a physician. This form must be answered completely and honestly. Failure to answer all questions may lead to revocation of certification. Take the Medical History Statement and the Medical Examination Report (Form F-2) to a physician, physician assistant, or nurse practitioner. Because you will be exposed to OC Pepper Spray and CS Tear Gas, have your physician sign the OC Pepper release form. Before you turn in your paperwork, you must sign an Authorization to hold medical records. This is required before I am allowed to keep any medical paperwork covered under the HIPAA Act. If you do not have a signed completed copy of this form I will not accept paperwork from you. You must provide a certified criminal record check from all locations in which you have lived since the age of 18. You must also provide a criminal history from any county in which you did not live and were arrested or charged with any crime. This may be a lengthy process depending on how many places you have lived. These must be obtained from the Clerk of Court in each of the counties in which you have lived. To download a request letter click here. If you are prior service military click here for the addresses of military criminal records. These can take a long time so do it early. You must provide a copy of your drivers history for any state you have lived in since the you have been driving. Follow this link for information about getting a Complete Extract Copy of your North Carolina Drivers History. It is your responsibility to have these documents BEFORE the class starts. DO NOT have a copy sent to the school or to me. DO NOT have a friend who is in law enforcement run a copy of your drivers history on North Carolina Division of Criminal Information (DCI) or National Crime Information System (NCIC). It is a violation of federal law for anyone but a criminal justice official to be in possession of these documents. SCHOOL TRANSCRIPTS OR DIPLOMA Provide transcripts OR diplomas from High School and Colleges you attended. Official transcripts are not required. Copies of transcripts or diplomas are acceptable. You will need photo copies of the following documents. 1. Drivers License 2. Birth Certificate or Proof of U.S. Citizenship 3. Social Security Card You must be sponsored by a law enforcement agency in North Carolina. To gain sponsorship, contact your local police department or sheriff’s office and ask them to sponsor you. Sponsorship does not mean they will pay for your books or tuition. It does not mean they will hire you when you finish. It means that they have checked your background and found nothing that will preclude you from becoming an officer. All applicants must write a 3-5 page paper. The paper should discuss all of the following points.
